Transferrin – what is this test?

Transferrin is a protein found in plasma. It is a glycoprotein that binds iron, usually saturated at 20-40%. A transferrin concentration test is essential for diagnosing various disorders and diseases, including:

• iron deficiency,

• hemochromatosis, which leads to excessive iron accumulation in the body,

• liver diseases,

• nephrotic syndrome.

Determining whether transferrin levels are elevated, normal, or low, as well as simultaneously measuring ferritin levels, will better assess iron metabolism than directly measuring the element itself.

Transferrin Test – Indications

The transferrin test is performed at the request of a doctor. It should be conducted to determine the causes of iron metabolism abnormalities that may manifest as:

• feeling unwell,

• quick fatigue,

• concentration problems,

• dizziness,

• feeling palpitations.

Transferrin in blood – preparation for the test

For the transferrin test, you need to go on an empty stomach. You should refrain from eating for 8–12 hours before the sample collection. Alcohol consumption and smoking should also be avoided, while proper hydration is necessary. It is recommended to drink about 2 liters of still water the day before the test.

How is the transferrin test performed?

The transferrin test should be done in the morning hours. A blood sample is collected, usually from the cubital fossa. The entire procedure, including disinfecting the skin and applying pressure at the collection site, takes a few minutes. It is not painful, although some patients may experience slight discomfort during the needle insertion.

Transferrin – normal values and results

The test results are interpreted based on the transferrin normal values. The normal transferrin concentration in blood serum ranges from 2 to 4 g/l. If a high transferrin level is detected, it may indicate anemia caused by iron deficiency. Another cause could be estrogen therapy. Low transferrin levels may be due to nephrotic syndrome, chronic liver diseases, or inherited metabolic disorders.
